- Unsigned. Most of the surface occupied by an extensive inscription in classical majuscule, now hardly legible due to wear and tear and cracking across with large stone losses. - The plaque commemorates Matthias von Carnitz of Niedysz (Neides), who died on 14 October 1628, son of Caspar von Carnitz and Anna Emma von Kleist, and his wife, also of the von Carnitz family. Matthias von Carnitz was a courtier, court councillor and later chancellor to Duke Philip II, on whose behalf he attended the Reich Parliament in Regensburg (Regency) in 1613. He is depicted in a well-known painting by Anton Mozart as one of the figures participating in the handover of the septenary to Prince Philip II in 1617. He probably built the late Renaissance manor house in Niedysz.

- Plate of standing rectangular form. The upper part shows the double coat of arms of the von Karnitz family (shields in the upper part with a representation of a stag, in the lower part - with a chequered pattern), in the lower corners - round rosettes.
